Who Is Eligible
- A person with a disability who cannot independently board, ride or get off a bus.
- A person whose disability-related condition or environmental barrier makes it impossible to independently travel to or from a fixed route bus stop.
These are the only criteria used to determine paratransit eligibility. Personal safety, behavioral problems, age, social security status or other issues are not considered in determining eligibility.
How Soon Can I Use Transit Plus?
You may begin scheduling rides on the day after you are determined to be eligible.
How to Apply
To apply for our Transit Plus service, please follow these steps.
Step 1: View, download and print two forms: (1) Transit Plus Eligibility Application and (2) Transit Plus Medical Documentation Form. You will need Adobe Acrobat Reader to view these forms.
Step 2: Complete your Transit Plus Eligibility Application. You then can either:

Or, you can bring your completed application with you at the time of your assessment.
Step 3: Have your medical provider complete the Medical Documentation Form.
Step 4: Schedule an assessment by calling (414) 343-1700. TTY number for hearing impaired in (414) 343-1704.
Step 5: Attend your assessment. Please arrive at least 10 minutes before your appointment.
Remember to bring the following to your assessment:
- Your completed and signed Transit Plus Eligibility Application
- The completed Transit Plus Medical Documentation Form
- A current form of photo identification
The Transit Plus office is in the Milwaukee County Transit System's Administration Building at the corner of N. 17th Street and Fond du Lac Avenue. There is parking available by the front door.

Travel Policies
You can go anywhere within Milwaukee County as well as to some limited areas of Waukesha and Ozaukee counties.
- Rides are available from 4:30 a.m. to 1:00 a.m.
- Cost is $4.00 for each van ride, with the exact amount paid in cash or by using a ticket.
- You may take another passenger with you for an additional $4.00 each way.
- Personal Care Attendants ride free.
- Please note that, depending on your destination, the entire cost of a taxi trip may not be covered in full by the $4.00 rider fare.

Rights & Responsibilities
All Milwaukee County Transit Plus passengers have a right to:
- A safe, comfortable, and clean vehicle
- A courteous and knowledgeable driver
- Equipment that has been inspected and drivers who have received training in the safe and effective use of lifts, ramps and restraints
- Service provided on a timely basis
- Easy access to current Transit Plus policies and procedures
- Prompt response to all comments and concerns regarding Transit Plus issues
- Be treated with dignity and respect by all Transit Plus employees
All Milwaukee County Transit Plus passengers are responsible for:
- Respecting other passengers and Transit Plus employees
- Paying fare as required, and showing proper identification upon request
- Providing a safe, accessible path of travel from the door of residence to the vehicle
- Cancelling scheduled rides in a timely manner
- Making any special requests/instructions at the time of reservation
- Understanding that service delays may result from circumstances that are beyond the control of the operator (traffic delays, inclement weather, vehicle breakdown)

Service Management
Transit Plus is managed by the Milwaukee County Transit System.
- First Transit and Transit Express provide ADA accessible van service as contracted carriers.
- American Logistics and American United are dispatch services that receive orders for taxicabs and attempt to dispatch these orders to self-employed taxi drivers.
- Taxis provide same day service to Transit Plus under a separate contract.
