How to apply for Transit Plus Services:

The fastest way to start the process of applying for Transit Plus services is to fill out the electronic application below:

Applicants or their designee are responsible for completing the Application.
Only licensed healthcare professionals are qualified to complete the Medical Verification Form.
Return the completed Application and Medical Verification form to the Transit Plus Office either in person or by mail.
The Transit Plus Office is open Monday – Friday (8:00am to 4:30pm) and is located at 1942 N. 17th Street, Milwaukee, WI 53205.
It is the responsibility of the person applying for Transit Plus Service or their authorized designee to ensure that all required elements of the application are complete.
Failure to do so will only delay the process of determining an applicant’s eligibility.
After the Transit Plus Office receives a complete application, the client or their designee will be contacted with available appointment times for a required in-person functional assessment at the Transit Plus office.
Transportation can be arranged by the Transit Plus Office for the assessment if needed.
Please note that any applicant who may require personal care, toileting assistance or behavioral supervision must be accompanied by a caregiver or family member capable of attending to their needs.
